Melena Revealing an Attenuated Familial Adenomatous Polyposis in an Elderly Patient: A Case Report

چکیده
We describe a patient who was diagnosed with multiple tubulleuvillous adenomas focus of high-grade tubular dysplasia all over the colonic mucosa, discovered during colonoscopy performed an episode melena. Genetic testing has identified germline truncating mutation at codon (5q22.2) adenomatous polyposis (APC) gene. This is localized in alternately spliced region exon 12, which associated attenuated familial (PAFA) phenotype. Our had no extracolic manifestations PAFA and none her relatives history rectocolic polyposis. Treatment consisted colectomy ileorectal anastomosis. ill-defined condition unknown prevalence penetrance, requiring individual treatment lifelong monitoring. It essential to identify these patients view setting up appropriate endoscopic surveillance early age family members carrying this mutation, due marked intra-family phenotypic variance.

Attenuated Familial Adenomatous Polyposis
Attenuated familial adenomatous polyposis (AFAP) is a subtype of a condition known as familial adenomatous polyposis [2] (called FAP or classic FAP). People with FAP or AFAP will have an increased number of adenomatous colon polyps during their lifetime and an increased risk of developing colorectal cancer [3]. An adenomatous polyp is a lump filled with the cells that make mucous and line the i...
متن کاملIdentification of an APC Variant in a Patient with Clinical Attenuated Familial Adenomatous Polyposis
Introduction. The objective of this case report is to discuss an unclassified germline variant of the adenomatous polyposis coli (APC) gene identified in an older patient with attenuated familial adenomatous polyposis syndrome (AFAP). Methods. We present a case report of a 66-year-old man diagnosed with AFAP. Colonoscopy found multiple polyps and invasive adenocarcinoma arising in the transvers...

A 67 year old man with a clinical diagnosis of attenuated familial adenomatous polyposis (AFAP) and a past history of synchronous colon cancers in the transverse colon was also found to have an intraductal papillary mucinous neoplasm (IPMN) of the pancreas. In addition, several foci of heterotopic gastric oxyntic mucosa were noted in the duodenum, interspersed with flat and polypoid adenomas. T...
متن کاملAmerican founder mutation for attenuated familial adenomatous polyposis.
BACKGROUND & AIMS Specific mutations in the adenomatous polyposis coli (APC) gene can lead to an attenuated form of familial adenomatous polyposis (AFAP). Although AFAP mutation carriers have a 69% risk of colorectal cancer by age 80, clinical recognition remains a challenge in some cases because they present with few colonic adenomas and are difficult to distinguish clinically from patients wi...
متن کاملAttenuated familial adenomatous polyposis presenting as ampullary adenocarcinoma.
The risk of periampullary cancer in patients with classic familial adenomatous polyposis (FAP) is significantly increased compared with the general population. However, the incidence of this extracolonic manifestation in attenuated FAP (AFAP) is unknown. We report the case of a 38 year old woman with no known family history of polyposis or colorectal cancer, who presented with ampullary adenoca...
